Visual Studio Port

 I don't think anybody has reported a working port, but we would certainly
welcome patches that would help people compile it with Visual C++.
I know some people have tried and reported name conflicts (winnt.h apparently
defines FIELD_OFFSET), but I am not sure whether anybody has been successful
so far.
